If you are a new applicant, a non-U.S. citizen, or a transfer from another state, you must provide proof of lawful status in the United States in the form of a certified U.S. birth certificate (state certified only; photocopies and hospital-issued certificates will not be accepted), a valid, non-expired U.S. Passport/Passport Card, a Certificate of Citizenship/Naturalization, a Permanent Resident Card, an Employment Author If your name differs from what appears on your proof of lawful status document, you’ll need to demonstrate the progression of each name change by submitting a certified copy of a marriage certificate (from a government entity, not the church), a divorce decree, and/or a court order for each name change. We can only accept genuine certified documents; photocopies will not be accepted.

If you are a new applicant or a transfer from another state, and your current driver licence does not have a gold star in the upper corner, you must also provide your Social Security Card, a W-2, a 1099, or a pay stub. All of which should show your social security number.
